The Primary Care Coalition of Montgomery County, Maryland was founded in 1993 when a group of physicians and health care officials imagined a Montgomery County where every resident had access to high-quality health services. They envisioned a dependable source of primary care for low-income, uninsured individuals in our community and created a framework that shared the responsibility for providing that care across the public and private sectors. The Primary Care Coalition (PCC) was formed to help make their vision a reality.

The PCC is deeply connected to the continuum of care for low-income uninsured, ethnically diverse residents of Montgomery County who frequently lead medically and socially complex lives. The PCC strives to provide a coordinated network of high-quality care for vulnerable residents of Montgomery County. The PCC works with the Montgomery County Department of Health and Human Services (DHHS), 12 independent safety-net clinics, six hospitals, and a number of other community partners to help patients’ access the full range of care they need.

Position Description

The Program Coordinator is responsible for administrative and project management support primarily to the team for the Nexus Montgomery Regional Partnership, LLC (Nexus Montgomery).  Nexus Montgomery is a collaboration of the six hospitals in Montgomery County, Maryland, focused on community based initiatives to improve or maintain health status among targeted population and reduce future hospital utilization. 

The Program Coordinator will:

  • Staff all Nexus Montgomery Board and Committee meetings, documents meeting minutes and deliverables, and is responsive to board member requests.
  • Maintain primary focus on program coordination and documentation of key activities.
  • Maintain a program schedule and provides the team with workflow support and coordination.
  • Develops and maintains positive working relationships with all contacts internal and external to the project(s).
  • Maintains a high-level of confidentiality and discretion due to the sensitive nature of the information.
  • Manage multiple responsibilities, with a high attention to detail and self motivation.

Responsibilities

  • Provide analytical, research, and project management support for Nexus initiatives.  This may include performance tracking, regulatory and policy analysis, and ad-hoc research supporting key initiatives.
  • Follow up on board and committee requests and facilitate sharing of information and analytics across hospitals, partner organizations, and other stakeholders.
  • Collaborate with program staff, hospital and community partners, and other stakeholders to ensure accuracy of data analysis and reporting, prove efficacy of population health programs, track monthly performance measures, and evaluate potential new programs.
  • Maintain a detailed project schedule which includes checklists and timetables to meet project deadlines and notify project team of approaching deadlines in a timely manner.
  • Plan and coordinate meetings and events to support the program(s), including Nexus Montgomery board and committee meetings.
  • Act as recording secretary to the program(s), preparing agendas and minutes, collecting information, and assembling materials for distribution.
  • Maintain accurate and complete electronic and hard copies of project files and data.
  • Meet organizational informational needs by providing records, reports, correspondence or other specific information.  
  • Provide administrative support to the Program Director(s) in day-to-day project activities.
  • Coordinate logistics and develop reporting materials to support project initiatives.
  • Participate fully and collaboratively in staff and team management work groups.
  • Interact harmoniously and effectively with others, focusing on the attainment of organizational goals and objectives through a commitment to teamwork.
  • Keep current on all professional and organizational training.
  • As requested, represent PCC and Nexus Montgomery at various community meetings in a professional and informed manner, resulting in the maintenance and/or establishment of key community and business contacts and connections.
  • Ensure all organization activities and operations are carried out in compliance with local, state and federal regulations and laws governing business operations.

Requirements

  • Bachelor’s Degree required.  Master Degree or classes helpful.
  • 2 years of professional work experience, preferred.
  • Strong interpersonal skills and the ability to interact effectively and build collaborative relationships with diverse clients, stakeholders and staff members at all levels.
  • Demonstrated understanding of health care, health care markets, hospitals, and population health strategies.
  • Ability to manage time effectively and demonstrate good problem solving skills.
  • Strong computer skills in Microsoft Office, Word, Excel and Power Point.
  • Ability to prioritize work and allocate limited resources resulting in high quality, customer-focused projects, meetings, forums and outreach efforts.
  • Excellent organizational skills with the ability manage multi-faceted projects while modeling commitment and enthusiasm.
  • Ability to write and organize reports with skill in gathering required information.
  • Excellent oral and written communication skills and the ability to communicate effectively with a broad range of stakeholders including clients, clinicians, hospital administrators and public health officials.
  • Strong listening skills, with an ability to capture and document salient discussion points and action plans that arise from board, committee, and other discussions.
