Primary Care Coalition of Montgomery County (PCC) in collaboration with the Asian American Health Initiative (AAHI) seeks a highly organized and motivated individual for the position of Senior Wellness Program Coordinator[CP1] . AAHI is part of the Montgomery County Department of Health and Human Services (HHS). Job Overview
The Asian American Health Initiative launches a new program to assist seniors and their families in improving health, wellness and quality of life.  The program focuses on health and wellness, senior safety and resources. The Senior Wellness Program Coordinator will play a central role in the development and coordination of the deliverables.  The position requires public health knowledge and experiences, high level program planning, implementation, evaluation, curriculum design as well as excellent analytical, interpersonal, organizational, and writing skills, along with the exercise of extensive discretion, independent judgment and political acumen. 
 
 
Responsibilities
  • Plan, organize, coordinate and evaluate complex program activities and diverse volunteers.
  • Coordinate and facilitate educational programs for seniors regarding health, safety and resources.
  • Educate seniors on available resources and application procedures.
  • Conduct outreach to seniors.
  • Develop and maintain a database of senior groups and resources.
  • Liaise AAHI with providers and stakeholders.
  • Collect, analyze and evaluate data and information, and make appropriate recommendations.
  • Establish and maintain effective working relationships with a variety of individuals and groups.
  • Provide administrative support to AAHI as appropriate.
  • Collaborate with HHS’s service areas and programs.
  • Provide support to HHS’s Aging and Disability Resource Unit.
  • Represent AAHI to participate in workgroups.
 
Requirements
  • A Bachelors Degree in Health, Social Work, Gerontology or equivalent amount of experience. 
  • 2+ years’ related experience.
  • Bilingual in English and at least one Asian language.
  •  “Eye for detail” to ensure error –free work product
  • Proficient with Microsoft Office applications (Excel, Word, PowerPoint); proficient with mobile devices and social media.
  • Experience working with diverse populations is a plus.
  • Ability to manage multiple tasks, prioritize assignments and plan ahead.
  • Ability to work well as a team member and function effectively under pressure.
  • Excellent verbal and written communication skills.
  • Must demonstrate standards of professional conduct, including dependability, reliability, and be able to communicate respectfully and appropriately with co-workers, colleagues, community contacts and advisory group members from diverse backgrounds.
  • Knowledge of basic principles of communication strategies and use of social media.
  • Ability to work during evenings and weekends as needed.
  • Inter-county travel is required.
