Telework: The job duties require in-person meetings and up to four days of in-person work (two-three days at health center sites, one day at PCC, and remaining in-person time on meetings at PCC or completing community- based visits with patients or other agencies.) Incumbent is also expected to be in the office to attend designated face-to-face meetings and perform in-office duties as assigned. The job duties require in-person presence to foster strong collaborative relationships and consultation with PCC and health center colleagues, as well as seeing patients in person. This role is approved for telework, including telehealth with patients, when activities do not require a physical presence. Note: PCC employees must reside in the following five states and District of Columbia: MD, VA, WVA, DE, PA. & DC. Relocation costs are not provided.

Summary: 

Primary Care Coalition (PCC) has an open position within the Montgomery Cares Behavioral Health Program (MCBHP) for a Bilingual Community Resource Coordinator. The BCRC helps connect underserved patients to health and social services, supporting their basic needs. This role is part of PCC’s Collaborative Care model, which integrates primary care, behavioral healthcare, and social support. The BCRC helps patients navigate health and social services, connects patients to resources, conducts outreach and research on resources, and assists team members with behavioral health related tasks and resource-related questions.

Essential Duties

1.) Provide Case Management services 
  • Support behavioral health care managers by providing case management services to patients as part of their treatment plans. 
  • Refer patients to community or social service agencies.
  • Complete home and community-based visits as appropriate, such as accompanying patients to county government offices to apply for benefits or deliver food from referral sources such as Manna to home-bound patients (advanced age, illness, or disability) who cannot access transportation.
  • Assist with benefit applications (e.g., SNAP).
  • Document all patient encounters appropriately in the Electronic Health Record and the case management system accurately and in a timely manner.
  • Maintain compliance with HIPAA.
2.) Collaborate with partnering agencies 
  • Maintain positive relationships with healthcare and social service providers.
  • Participate in community events and networking.
  • Act as a patient advocate and liaison.
  • Share information with MCBHP team members on community resources that are available to patients.
  • Serve as the point of contact and manage referrals for the MCBHP for programs that accept referrals from MCBHP, such as for the annual Holiday Giving Program.
3.) Support Care Managers with providing behavioral health interventions
  • Assist Care Managers with behavioral health screenings for anxiety and depression
  • Support patients with health literacy challenges (e.g. teaching patients how to make follow up appointments at primary care clinics)
  • Support recommended behavioral health changes by providing basic nutrition information and lifestyle education and counseling.
  • Provide support to patients and providers during primary or specialty care visits.
  • Serve as back-up support to interpret for psychiatric clinic when Psychiatric Services Coordinator on leave.
  • Attend trainings to improve skills in appropriate areas and learn about new resources available for patients
  • Contribute to program publications and PCC committees.
  • Participate in meetings of the MCBHP, PCC, and health center staff as appropriate.
  • Other duties as assigned.
Skills and Abilities

· Bilingual English and Spanish Required
· Team player with strong interpersonal skills.
• Effective time management and problem-solving abilities.
• Knowledge of community resources and benefits and ability to coordinate referrals.
• Proficient or able to learn and utilize electronic health records and Microsoft Office.

Education and Experience 

· High School graduate or GED required
· Minimum 2 years’ experience in health or social services (3+ years preferred).
• Experience with diverse, low-income, immigrant populations.
• Knowledge of behavioral health or medical issues preferred.
• Community Health Worker licensure preferred.
