Telework: The job duties require in-person meetings and up to three days of in-person work (two days at assigned health center site/s and remaining in person time on meetings at PCC or visiting partner sites or other agencies. Incumbent is also expected to be in the office to attend designated face-to-face meetings and perform in-office duties as assigned. The job duties also require in-person presence to foster strong collaborative relationships and consultation with PCC and health center colleagues, as well as seeing patients in person. This role is approved for telework, including telehealth with patients, when activities do not require a physical presence.  Note: PCC employees must reside in the following five states and District of Columbia: MD, VA, WVA, DE, PA. & DC. Primary Care Coalition has an open position for a Bilingual Supervisory Behavioral Health Care Manager who is responsible for offering behavioral health services to underserved patients living in Montgomery County, as well as directly supervising other staff and collaborating with Director on quality improvement and assurance. This position is best suited for those who are passionate about health equity and enjoy collaborating with other providers to offer behavioral health services in a setting that is integrated with primary care.  The Bilingual Supervisory Behavioral Health Care Manager provides clinical services directly to patients and also collaborates closely with the Director and other Supervisor on improving the patient  and stakeholder experience.
Approximately 75% of this role is dedicated to direct patient care, and 25% on supervisory/leadership duties.

Reporting to the Director of Behavioral Health, you would be assigned to work in a hybrid model at one or more of our safety-net primary care clinics.  You would also be responsible for leading a team of up to four direct reports, consisting of behavioral health care managers (mental health clinicians or nurses with behavioral health experience and/or Community Resource Coordinators.)  
As a team member within the Collaborative Care model, you would support primary care providers by identifying, treating and tracking patients with common behavioral health problems such as depression and anxiety. In addition to being part of a multidisciplinary medical team, you would also be part of a broader behavioral health team supporting patients across Montgomery County.  Our Collaborative Care model makes behavioral health care more accessible for those who need it most. As a Bilingual Supervisory Behavioral Health Care Manager, you will be part of a team that is working to advance health equity for vulnerable patients.

This job is best performed by someone who has an open and flexible mindset, is a self-starter, and is eager to contribute and learn.  While this is a clinical position, the Care Manager should be flexible in providing “outside of the box” interventions (such as case management, brief psychoeducation, and care coordination with medical providers).  Additionally, the candidate should be comfortable flexing communication style and approach to engage with leadership and other staff at the health centers.   As the primary patient population consists of immigrants from Central and South America, fluency in Spanish and English are required.  The position is perfect for someone who prefers regular business hours (no on-call requirement).

Essential Duties

1) Provides clinical services, utilizing the Collaborative Care model of integrated behavioral healthcare
  • Perform and document biopsychosocial evaluations, seeks psychiatric team consultation, and develops and implements treatment plans with patient
  • Reassess and monitor patients’ response to treatment on a regular basis using program instruments to track patient response to medication and/or other treatment
  • Provide psychoeducation, behavioral activation and therapy to patients on an as needed basis as part of their treatment plan
  • Identify appropriate resources, and coordinates referral process to community resources or behavioral health resources to meet their treatment goals
  • Provide on-site crisis management and referrals, when necessary, in consultation with primary care providers, Supervisory Behavioral Health Care Manager, Program Director and consulting psychiatrist
2) Serves as representative of behavioral health at assigned site/s
  • Ensure that the Behavioral Health Program functions effectively and efficiently at the assigned primary care clinic site/s. Ensures that Behavioral Health Program Operating Procedures are implemented and followed
  • Serve as the main liaison between the BHP and the partner clinic, maintaining positive and productive relationships with clinic staff and other key people
  • Oversee the productivity of the BHP at the host site, and when appropriate take measures to ensure that access to services is available to patients and that target goals for serving patients are met
  • Serve as the primary liaison with the psychiatric consultants, patients, and the primary care provider regarding patient care
  • Assist assigned sites with workflow and policies on crisis intervention and other behavioral health topics
3)  Leads others: Creates a cohesive and productive work environment in order to achieve the required outcomes.
  • Drives culture focused on exceptional customer service, efficiency, satisfaction of the clinic partners and patients.  Areas include, but are not limited to:   
  • Hires, supervises, oversees on-boarding and develops department staff, including clinically licensed individuals, to ensure the workflows accomplish the department’s goals and objectives  
  • Clearly communicates performance expectations, evaluates and provides direct reports with opportunities to develop skills and progress in their careers, and when necessary, implements and manages staff performance improvement plans. 
  • Ensures PCC policies and procedures are adopted and serves as a mentor and coach to direct reports willingly sharing knowledge and expertise and involving appropriate staff in planning, decision-making and problem solving.  
  • Escalates issues appropriately and seeks consultation from Director as needed to address team and program challenges. 
  • Problem solve/mediates when issues arise at assigned clinics
4) Collaborates on Quality Improvement, Assurance, and Stakeholder Engagement
  • Collaborate with the Program Director to identify needs and develop program policies and protocols, patient education materials, and training programs. 
  • Collaborate with Program Director to set annual program goals and objectives and metrics. Meet monthly with the MCBHP Director to report on progress at clinic sites, analyze data related to goals and objectives.
  • Provide and organize trainings for community partners and staff at partner clinics 
  • Develop and update templates for electronic health records, in collaboration with  Director
  • Meet with clinic administrative staff at each assigned site to track progress towards work plan and annual site goals. These meetings will include review of program data and reports provided by the MCBHP Program Director, identification of areas for improvement, development of processes to achieve behavioral health integration. 
  • Perform quality assurance reviews of MCBHP documentation to ensure that it is complete and follows guidelines and that care is being provided consistent with the program model of care. Prepare and deliver regular feedback to MCBHP staff on outcomes of quality assurance review and identified areas of improvement. 
 Additional Duties

• Participate in staff meetings of the Montgomery County Behavioral Health Program, partner health center sites, and Primary Care Coalition as scheduled. May also be asked to present on topics as needed- should be comfortable with preparing and giving public presentations, both virtually and in-person
• Attend appropriate trainings and meetings to improve skills in appropriate areas.
• May supervise interns or provide clinical supervision to graduate level providers as appropriate.
• Contribute to program publications and PCC committees, including case studies.
• Other duties as assigned, including providing coverage for other Care Managers if needed.
• Attend mandatory PCC Manager trainings
• Represent PCC/Healthcare Access Team/MCBHP team with internal and external stakeholders, including DHHS.
• Contribute to reports (including DHHS or other funders)

Education and Experience

LMSW, LGPC, LCSW-C, LCPC, Licensed Psychologist or BSN Required.  LCSW-C or LCPC preferred.
• Must have active (unexpired) Maryland Professional License for one or more items listed in the above bullet
• At least four years of professional experience (may include graduate school experience) working in mental health or social services with underserved populations
• Lived or working experience, and sensitivity to, the needs of immigrant and other vulnerable populations
• Experience working in a leadership role preferred, which may include interns

Bilingual in English and Spanish required. Must have reliable transportation. LMSW, LGPC, LCSW-C, LCPC, Licensed Psychologist or BSN Required.
